We are an approved Medicaid provider in the state of Georgia. Our top priority at MC Elite Case Management LLC is to provide high-quality case management services to individuals enrolled in the Independent Care Waiver Program (ICWP). The ICWP is a Medicaid waiver program designed for Medicaid-eligible adults aged 21 and 64 who have physical disabilities or traumatic brain injuries (TBI) that satisfy nursing home eligibility requirements. We work closely with our clients and healthcare professionals to ensure that our clients receive the proper and necessary support needed to live safely in their homes rather than in a nursing home facility.
